Service Description
This form is used to request the creation or revision of a digital badge for an academic or professional event. The badge will be issued through Accredible and should reflect the achievement or participation of the recipient.
Faculty or staff may use this form to:
- Request a new badge or revise an existing badge
- Provide details about the event date, badge name, and badge description
- Specify the criteria for earning the badge
- Upload any logo or artwork to be included in the badge design
Once submitted, a team member will review the request and follow up with any questions or confirmation.
